feat: adapt MCP tools to operation registry

Summary

• route analysis MCP tools through operation registry descriptors and input shapes
• preserve MCP success/error envelopes, nextSteps hints, and special cases such as groups/detect_changes/impact errors
• add registry/MCP parity coverage and update roadmap/docs for the completed adapter slice

Validation

• pnpm lint
• pnpm typecheck
• pnpm build
• pnpm test (30 files, 440 tests)
• pnpm verify:cli-real (pass=56 fail=0)
• code-review artifact: /tmp/code-review-mcp-operation-adapter.json (CLEAN)

Canary

• no stable release; canary verification will run after merge
