Context

Before executing a heal, show historical success rate from memory store in the approval UI. Parent: #49.

Acceptance Criteria

• Approval gate UI shows: 'This fault type seen 5 times; restart_pod resolved it 4 times'
• Context sourced from memory store query API, not hard-coded
• If no history exists, UI shows: 'No prior incidents matching this fault pattern'
• Memory context visible in both human-gate approval modal and auto-approve log
