Context

Existing Phoenix dashboard components refactored as a lens panel for the Sentinel unified shell. Parent: #46.

Acceptance Criteria

• Phoenix exports a ReliabilityLens component mountable by the Sentinel shell
• Lens renders the swim-lane pipeline (detect → reason → act → approve → verify)
• Lens reads from world model and event bus APIs, not local state silo
• Standalone Phoenix app still works for local development
